SCLAD was born in Hialeah, Florida in 1985, for the purpose of improving the opportunities of persons with physical disabilities to lead a full, inclusive life.

SCLAD's founders knew from personal experience, that access at home; access to basic daily living services and access to their community, are the keys to opportunity, residential stability and inclusion.

THE NEED


Persons with disabilities have diverse needs, depending on the type and severity of their disability. For many, a set of personalized supports is needed in order to obtain a successful outcome and achieve community integration. These needs may be as simple as securing Food Stamps or as difficult as obtaining housing that is appropriate.
For those with the most severe disabilities, there are vital needs that have to be fully addressed in order for them to enjoy “community living.” However, there are significant obstacles that keep too many persons with disabilities isolated or institutionalized.

SCLAD tries to assess each person's needs and, together with the individual, implement an approach that is conducive to achieving the benefits of community living.

With constant changes in Medicaid, Medicare, the Affordable Care Act and health insurance coverage in general, people with disabilities find themselves dealing with a system that concentrates on medical issues when most disabilities are not medical in nature.

SCLAD's Case Managers help their clients navigate through this complicated system; advocate on their behalf when necessary; and, coach them on how to understand and work with the system to secure the services they need to accomplish their inclusion goals.
