The support services offered by
SCLAD are designed to create and/or strengthen a 'support network' which serves
as the foundation for attaining full integration and independent living.
Services include: Case Management,
Life Skills Training, Information & Referral, Individual and Group
Counseling, Family Support, Basic Needs Assessments, Emergency Preparedness,
Home Visits, Telephone Reassurance, Housing Assistance; and, coordination of
Personal Assistance, Medical, Child Care and Transportation Services. SCLAD also helps individuals obtain any other item or service they may need to
achieve their independent living goals.
Each customer's Individualized
Service Plan (ISP) sets in place this 'support network' which is 'designed and
built' to be sufficiently reliable so that the individual can commit to a
training and/or work schedule, and sufficiently affordable so that it does not
create a financial burden that results in a disincentive to employment.
